Q: What do we do?
A: Provide 24-hour supervision in an independent apartment
    Teach and assess independent living skills, social skill, and areas of risk
    Develop individual programming to meet consumer's needs (ISP)
    Stimulate activities within the community
    Coach consumers through risk management and other areas of concern
    Improve life skills
    Support consumers to begin working toward life long learning goals

Q: How do we do it?
A: Licensed Waivered Service Provider
    Fully trained coaches who interact and assist consumers with living skills
    24-hour plan of care
    Funding private, MR/RC. CADI, TBI Waivers

A: Housing with supports
    Med administration
    ISP supports and development of independent living skills
    Rep payee
    Financial planning
    Medical and mental health visits
    Outside social activities
    Coordinate with work or school
    Psychiatric follow-up
